Palliative Care and Hospice Information Session

What would you do if you were diagnosed with a chronic disease, or worse, a terminal condition? Who could you rely on to ensure that all of your appointments and medications or other treatments are organized and coordinated? Are you a veteran? Do you or your family need someone familiar with the health care system to talk to? Do you need someone to help you in the grieving process?

Palliative Care helps people have more control over their lives when they receive a diagnosis of an on-going condition, such as Diabetes, Kidney or Liver Disease, Depression and other Mental Health issues, Dementia, Pain, Sleeping Problems or other conditions. Hospice Care provides critical support at the end of life or when treatment has stopped.

Don and Kristin, from Crossroads Hospice and Palliative Care are here to help you learn how to best prepare for your care in the future, or to help you gain more control over your health access and decisions if you are currently dealing with disease of your own or of a loved one. Palliative and Hospice care services also include helping you determine the funding that you have available and may even be able to find you additional sources of funding to further protect your wellbeing.
